(ii) Letter of undertaking as per Annexure – A
(iii) Educational qualification including year wise details of GNM examination (year, total and acquired marks in both 1st & 2nd yr., percentage)
(iv) Proof of date of birth (Matric/ equivalent certificate)
(v) Caste Certificate for SC/ST/OBC (as applicable for the current year)
